Information Bulletin cum Application Form For Fresh/Renewal of Accreditation 2014 - Regarding
Attention: Head of the Institutes/Hospitals/Medical Colleges
It is hereby informed that the application seeking accreditation for Diplomate National Board (DNB) programme in various Broad/Super specialties can be submitted in the prescribed format as per the following schedule:
Applications seeking fresh accreditation:

Latest by 31st January 2014
Applications seeking renewal of accreditation:
NBE Accredited Hospitals /institutions, accreditation of which is expiring in June 2014/ December 2014
Note:
1. Applicant hospitals /institutes are advised to read the instructions carefully and provide correct information only.
2. Incomplete application or application not in accordance with guidelines will not be considered.
3. Applicant hospitals /institutions are advised to kindly contact NBE by e-mail at accr@natboard.edu.in for queries, if any.
4. All payments towards accreditation fee are to be made through RTGS/NEFT or by Cheque along with Prescribed Challan only at all branches of Indian Bank or of Axis Bank. Please refer to the chapter 5 of information bulletin for details of Fee Schedule.
5. Applicant hospitals /institutions may kindly note that the use of any agent or agency or any party who is not an employee or office bearer of the applicant hospital /institution for the purpose of preparing, drafting, submitting and / or representing the applicant hospital / institution is strictly prohibited. In the event of such an instance been brought to the notice of NBE, the NBE reserves its absolute right to summarily reject application besides further action as may be deemed appropriate including but not limited to debarment from filing application seeking accreditation in future.
